(Ages 10 to 14 Years old)

 
The rules for this contest are simple – just share your unique
Halloween badge button designs and you could win!

First Prize - $10.00 McDonald Gift Certificate

Second Prize - $5.00 McDonald Gift Certificate

We’re looking for button ideas that:


  • Halloween Theme Scary, Full or Silly

  • Images, Photos,

  • Scary, Fun, or silly
(Button size is 2 1/4 )

If your design is selected as the “Halloween Badge Button of the
Month,” your entry will be included on our website and/or in our
online newsletters, advertisements or printed catalogs
 
Judging:  Winners will be selected by a panel of judges. Judges’
decisions are final. Winners’ names will be announced on
Prevention Force Family Center website at www.pffc.blogspot.com
and prizes will be present to winners at the Fredrick Douglass
Library on Friday, October 30, 2015.   Acceptance of prize
constitutes permission for Prevention Force Family Center to use
the winner’s name and/or likeness, and the winning button
artwork/photos for purpose of advertising and trade without
further compensation. Artwork may be altered or enhanced for
use in our promotion. No substitution of prizes allowed.

All entries are due by Friday October 23, 2015 at 4:00pm at Douglass Library Branch - 3353 W. 13th Street, Chicago, Illinois 60623 or Prevention Force Family Center - 4728 Madison Street, Chicago Illinois 60644.

Prizes will be present to the winners at the Fredrick Douglass Library on  Friday, October 30, 2015 at 4:00pm.

·         Only 1 Halloween Badge Button Design per child can be submit. Design must stay inside the 2 14 circle.

·         Halloween Badge Button must be create on this form or go to button biz website at  http://buttonbizdesigner.com/main.php (must register and click on badge button 2 ¼ button size and create your button design in print it and attach to this form).

·         All entries must be submit by  Friday October 23, 2015 at 5:00pm to Prevention Force Family Center 4728 W. Madison Street, Chicago IL 60644.

TEACHING HOW TO SAVE LIVES






























Jenette Cosey and Amanda Ball are making a difference on Chicago’s West Side and in the lives of the individuals in numerous communities. As Senior Community Service Employment Program (SCSEP) participants, they are receiving valuable training at Prevention Force Family Center. The training they are receiving will assist them in achieving their professional goals.

Mrs. Latonya Clark, Client Service Representative at (SCSEP)  recently had an opportunity to watch Jenette and Amanda in action. She said,"I was truly impressed with these awesome women!" Even though it was Jenette’s first time facilitating a CPR Life Skills class she did a wonderful job. Jenette was previously a home healthcare worker for numerous years. Her experience in the healthcare field assisted her in transitioning to the CPR Life Skills Facilitator training position at Prevention Force Family Center..

Amanda assisted Jenette every step of the way. Amanda was previously a registered nurse for numerous years. Her experience in the healthcare field assisted her in transitioning to the Administrative Assistant position over the  Health and Youth Department at the agency. Amanda assist CPR instructors with the CPR/AED and First Aids training classes. Amanda is always willing to help a team member in need.

Mrs. Clark said, "It was great to witness teamwork. It takes a team to achieve program goals. At Prevention Force Family Center, SCSEP participants are learning the true meaning of teamwork. Mrs. Clark said, She was proud that they were able to work as a team when it mattered the most." As she observed Jenette and Amanda teach individuals in the class, She was excited for them, as they taught others how to keep the heart pumping for individuals who are in distress. Congratulations to both Jenette and Amanda for being a true example that learning is powerful!
